National Hospice Palliative Care Week is a time to celebrate and highlight the incredible work being done to provide quality palliative care across the country. This year’s theme, “Palliative Care Everywhere”, reminds us to work together for better access to quality hospice palliative care, for every community, in every setting. The first full week of May marks National Hospice Palliative Care Week in Canada each year.
The Hamilton Family Health Team hosts a Palliative Care Coach on behalf of our Ontario Health Team (OHT). This role collaborates with health and community care partners to assess educational needs related to a palliative approach to care. Education that is provided in different settings helps to reinforce that palliative care is most effective when integrated early and consistently across the trajectory, not limited to end-of-life care.
