Your Opportunity:

The Patient Experience Specialist is responsible for building, implementing, and continuously improving HFHT’s patient relations and patient experience programs. This role is the primary point of contact for patients, families, and caregivers navigating concerns, complaints, or compliments about their care — and a key driver of how HFHT listens and responds at an organizational level.

You will work collaboratively with physician affairs, clinical managers, and primary care practices to co-design patient relations processes that are consistent, equitable, and person-centered. You will bring data to life through quarterly trend reports and an annual patient experience report, and you will play an active role in HFHT’s Patient and Family Advisory Group — including leading the planning of Patient Experience Week each year.

Roles and Responsibilities:

Patient Relations

  • Serve as the primary contact for patients, families, and caregivers with concerns, complaints, or compliments, ensuring responses are timely, compassionate, and resolution-focused in accordance with the Excellent Care for All Act, 2010.
  • Manage the end-to-end patient relations process — intake, acknowledgement, investigation, resolution, and follow-up — within legislated timeframes and organizational policy.
  • Co-design and evolve the patient relations process in collaboration with physician affairs and clinical managers, ensuring consistency across HFHT’s sites and practices.
  • Apply equity, diversity, and inclusion principles in all interactions, ensuring culturally sensitive and responsive engagement with the diverse communities HFHT serves.
  • Liaise with primary care practices to support concern resolution at the practice level, coaching staff and providers on service recovery and conflict resolution.
  • Identify and surface themes from patient feedback to inform quality improvement and organizational learning.

 Patient and Family Advisory Group

  • Support the coordination and facilitation of the Patient and Family Advisory Group (PFG), including meeting preparation, agenda development, and action follow-up.
  • Build trust-based relationships with patient and family advisors, supporting their meaningful participation in organizational work.
  • Lead the planning and execution of Patient Experience Week in partnership with the PFG.
  • Identify opportunities to embed patient and family voice into quality improvement, program development, and organizational decision-making.

 Data Analysis and Reporting

  • Administer and analyze HFHT’s patient experience survey program, ensuring data accuracy and meaningful interpretation.
  • Develop and produce quarterly patient experience trending reports for the Quality Committee.
  • Lead the development of the annual patient experience report, synthesizing survey data, patient relations themes, and advisory group insights.
  • Support the Manager in preparing patient experience summaries for the Board Quality Committee and other governance audiences.

 Collaboration and Engagement

  • Work cross-functionally with clinical managers, physician affairs, and program leads to embed patient experience principles into operations and improvement work.
  • Support staff and provider capacity to respond to patient concerns with empathy, professionalism, and a focus on resolution.
  • Stay current with best practices through external networks including the Ontario Patient Relations Association and the local Ontario Health Team.

Qualifications and Skills:

  • Undergraduate degree in a health-related field, social sciences, communications, or a related discipline.
  • Three (3) to five (5) years of experience in patient relations, patient experience, health advocacy, or a related healthcare role.
  • Demonstrated experience managing patient complaints and concerns with a person-centred, resolution-focused approach.
  • Strong skills in service recovery, conflict resolution, facilitation, and negotiation.
  • Commitment to equity, diversity, and inclusion in practice; training in Indigenous Cultural Safety, unconscious bias, or allyship an asset.
  • Familiarity with relevant Ontario legislation, including the Excellent Care for All Act, 2010, the Quality of Care Information Protection Act, and the Apology Act.
  • Strong data analysis skills with experience producing clear, actionable reports for non-technical audiences.
  • Excellent written and verbal communication skills with the ability to engage effectively across all levels of the organization — from patients and families to governance bodies.
  • Strong organizational skills and the ability to manage multiple active files and priorities simultaneously.
  • Experience with patient and family engagement or advisory programs an asset.
  • Proficiency with EMR or patient experience survey platforms; familiarity with Ontario primary care systems an asset.

As a Mental Health Counsellor you will be part of the team in the fast-paced practices of family physicians. Your Opportunity:

The Registered Practical Nurse (RPN), Patient Attachment, is a mobile member of HFHT’s Attachment Team, working across all clinic sites to support the attachment of unattached and at-risk patients to primary care providers. This role is central to HFHT’s patient attachment strategy, functioning at the point of care to coordinate outreach, complete clinical onboarding, and facilitate warm connections between patients and their care teams.

The RPN is expected to travel between HFHT clinic sites on a regular basis and may work from various clinical and community settings. Work may include in-person, virtual, and telephone-based interactions. This position is a full-time role and will involve interactions with patients who may be experiencing social vulnerabilities. We are currently looking to hire 3 full time RPNs for HFHT’s Attachment Team.

Roles and Responsibilities:

Patient Outreach and Engagement

•       Conduct proactive outreach to unattached patients and those at risk of losing primary care access, via telephone, virtual platforms, or in-person visits.

•       Engage with patients in a trauma-informed, culturally responsive manner to support their connection to primary care.

•       Facilitate warm handoffs between patients, community partners, and care providers to support seamless attachment at the point of care.

•       Identify and address barriers to attachment, including social determinants of health, and connect patients to appropriate resources.

Clinical Onboarding and Chart Preparation

Your Opportunity:  
The Hamilton Family Health Team is seeking applicants for one full-time Registered Practical Nurses at the Healthcare at Eva Rothwell Centre located at the Eva Rothwell Centre, 460 Wentworth Street North, a trusted community facility since 2004, where social and community programs complement the activities of the clinic. 

Duties and Responsibilities: 

  • Chronic Disease Management Support 
    Assist in the monitoring and follow-up of patients with chronic conditions (e.g., diabetes, hypertension, COPD, heart failure) under the direction of the NP
  • Perform clinical tasks including vital signs, point-of-care testing, wound care, and specimen collection
  • Provide patient education on disease management, medications, and self-care strategies
  • Support recall and follow-up processes for patients with complex chronic disease needs 

 
Preventive Care, Well-Woman, Well-Baby, and Prenatal Care 

  • Support well-woman visits including routine health screening, Pap tests (as per scope and HFHT protocols), and reproductive health education
  • Assist with well-baby and well-child visits including growth monitoring, developmental screening, and caregiver education
  • Provide prenatal support including blood pressure monitoring, urinalysis, fundal height measurement, fetal heart rate auscultation, and prenatal education
  • Administer vaccines according to Ontario immunization schedules and HFHT medical directives, including documentation and cold chain management
  • Conduct immunization counselling and address vaccine hesitancy using evidence-informed approaches
  • Support recall and reminder processes for overdue immunizations and preventive screening 

Care Coordination and Navigation 

  • Assist patients in navigating health and social services, including referrals to specialist care, community agencies, and HFHT allied health providers
  • Support warm handoffs between team members, including social work, dietetics, and community health workers
  • Participate in care planning for high-complexity patients alongside the NP and interprofessional team
  • Liaise with the Eva Rothwell Centre’s community programs to support integrated care

Clinical Administration and Triage 

  • Conduct telephone and in-person triage, assess patient needs, and prioritize care in collaboration with the NP
  • Manage patient flow and prepare patients for clinical encounters
  • Follow-up on patient’s blood work and imaging results as per NP direction
  • Maintain accurate, timely EMR documentation in accordance with CNO standards and HFHT policies
  • Support medication management processes including prescription preparation and reconciliation tasks within RPN scope
  • Contribute to quality improvement activities, including data collection and chart audits
  • Responsible for ordering vaccines and clinical supplies, stocking clinical rooms, and sterilizing clinical tools 

 Qualifications and Skills:  

  • Current registration in good standing with the College of Nurses of Ontario as a Registered Practical Nurse
  • Completion of a recognized RPN diploma or nursing program
  • Current Basic Life Support (BLS/CPR) certification

The Nurse Practitioner (NP) at the Eva Rothwell Centre is a full-scope primary care provider within Hamilton Family Health Team’s NP-led model at this equity-focused site. The NP holds an enrolled patient panel and is responsible for the full continuum of primary care delivery — from episodic and preventive care through to chronic disease management, care coordination, and patient advocacy — for a population experiencing significant health and social complexity. 

The NP practices in accordance with the College of Nurses of Ontario standards for the Extended Class, HFHT policies and procedures, and applicable Ontario legislation. 

Roles and Responsibilities: 

  • Primary Care and Chronic Disease Management 
    Provide comprehensive, longitudinal primary care to an enrolled patient panel with a high burden of chronic and complex disease
  • Diagnose, treat, and manage chronic conditions including but not limited to diabetes, hypertension, COPD, heart failure, chronic pain, and mental health conditions
  • Develop and implement individualized care plans in collaboration with the patient and interprofessional team
  • Conduct regular reviews of medications, investigations, and care goals for patients with multiple comorbidities
  • Support patients in self-management, health literacy, and navigation of the health system 

Preventive Care, Well-Woman, Well-Baby, and Prenatal Care 

  • Provide well-woman care including routine health screening, Pap tests, reproductive health counselling, and contraception management
  • Deliver well-baby and well-child visits including growth and developmental surveillance, immunizations, and caregiver education
  • Provide prenatal primary care including initial assessment, ongoing monitoring, prenatal education, and coordination with obstetric and midwifery services
  • Administer and prescribe immunizations in accordance with Ontario immunization schedules, HFHT medical directives, and CNO standards
  • Conduct proactive outreach and recall for overdue preventive screening and immunizations within the enrolled panel

Episodic and Urgent Care 

  • Assess and manage acute and episodic presentations within NP scope, with appropriate escalation and referral
  • Conduct telephone triage and same-day appointments in response to urgent patient needs
  • Collaborate with the RPN and clinic team to manage patient flow and ensure timely, appropriate care

Care Coordination and Navigation 

  • Lead care coordination for high-complexity patients, facilitating communication across HFHT team members, specialists, and community partners
  • Initiate and participate in interprofessional care conferences and case reviews
  • Facilitate warm handoffs to social work, dietetics, pharmacy, community health workers, and Eva Rothwell Centre community programming
  • Support patients in accessing housing, income supports, legal services, and other social determinants resources in collaboration with the social medicine team 
  • Serve as a clinical point of contact for community partners including the Eva Rothwell Centre, Thrive Group, and McMaster University research collaborators

Documentation and Quality 

  • Maintain accurate, timely, and complete EMR documentation in accordance with CNO standards and HFHT policies

  • Contribute to the development and review of clinical protocols, medical directives, and care pathways relevant to the Eva Rothwell population

  • Master’s degree in Nursing or recognized NP education program

We are seeking a dedicated and skilled Physician Assistant to join our Family Medicine team on
a full time basis. The ideal candidate will provide high-quality patient care in a collaborative

Responsibilities:
• Conduct comprehensive patient assessments, including physical examinations and medical
histories.
• Assist in the diagnosis and management of acute and chronic illnesses.
• Administer medications and perform procedures as directed by supervising physicians.
• Collaborate with healthcare professionals to develop and implement patient care plans.
• Maintain accurate and up-to-date electronic medical records for all patient interactions.
• Provide education and support to patients and their families regarding treatment options and
health management.
• Engage in pain management strategies for patients requiring specialized care.
• Work effectively in a medical office setting, demonstrating excellent
communication skills with both staff and patients.

Qualifications:
