The Hamilton Family Health Team is a community of 1,000 dedicated family physicians, interdisciplinary healthcare providers, and support staff leading the way towards healthier communities within the Greater Hamilton Area.

As the largest family health team in Ontario, spread across 96 sites, our teams take pride in partnering with you on your health and wellness journey. We’re with you at every stage of life—from well baby checkups, through adolescence and adulthood, to end-of-life and palliative care—so your care grows with you. We also help you connect to other services in your local neighbourhood so you have care as close to home as possible.

A message from the outgoing Chair of our Board of Directors

Dr. Jordan Wronzberg, Chair, Hamilton Family Health Team Board of Directors

It has been a privilege to serve for the past two years as Chair of the Board of Directors for the Hamilton Family Health Team, and I can think of no better year to close this chapter than one defined by such tremendous growth.

In May 2025, we launched our new Strategic Plan, with a refreshed vision: “Leading the way to a healthier community through team-based primary care”. This plan places our patients, our providers and staff, and our community at the centre of our work.

Over the past twelve months, we have seen significant investment into primary care from the Province of Ontario, and in close collaboration with our Primary Care Network partners, we have attached thousands of new patients to the team-based primary care they deserve. Together, we have expanded access, strengthened referral pathways, and ensured that more Hamiltonians than ever have a place to turn when they need care.

As I step down from this role, I do so with immense gratitude. To everyone who made this year possible, thank you. Our community is healthier because of your work, and I have no doubt this organization will continue leading the way for years to come.

The Province of Ontario set an ambitious vision through the Primary Care Action Plan: to connect every person in Ontario to a primary care provider or team. It is a vision we share wholeheartedly, and one we are bringing to life locally.

We’ve structured this report using the three pillars of the plan: Connecting You to a Primary Care Team; Making Primary Care More Connected and Convenient; and Supporting Primary Care Providers.

This is work we cannot do alone.

We are grateful for our incredible family physicians and their practice staff, our interdisciplinary health care professionals and staff, along with our Greater Hamilton Health Network (GHHN) Primary Care Network partners. We have incredible people working within our Hamilton Family Health Team community: the People of Primary Care.

Together we are working to ensure those in our community who need a family physician or nurse practitioner and primary care team are attached to one. To have a team you can count on, who knows you and your family and is in your corner to help when needed is invaluable. We are equally grateful for the hundreds of community partners who have joined us in helping to reach those who are in need of a primary care team.

The stories you share with us fuel our passion: the relief you experience from finally having a family doctor or nurse practitioner after many years without care, and the appreciation for your care team when you need support. Your stories remind us why our work matters.

Thank you for your trust in the care we provide. You are the reason we do what we do, and you will continue to be at the centre of everything we build in the years ahead.
