Meeting patients where they’re at
The Hamilton Family Health Team and our Primary Care Network partners have been all across the Greater Hamilton Area to connect new patients to family doctors. We’re in vaccine clinics, hospitals, thrift stores, food banks, places of worship, community and recreation centres, libraries, and many more locations.
Patients do not need a fixed address or phone number to enter into our program. We are reducing barriers to getting connected so all residents can have the primary care they deserve.
