Your Opportunity:
We are looking for a highly organized and collaborative Project Coordinator to support the delivery of HFHT’s primary care integration and quality improvement initiatives. If you thrive on keeping projects on track, bringing clarity to complex work, and ensuring teams have the information and support they need to succeed — this is the role for you.
Roles and Responsibilities:
• Maintain project plans, schedules, action logs, and status dashboards across PCAT, team-based care expansion, and other strategic initiatives.
• Coordinate meetings and working groups — scheduling, agenda preparation, minute-taking, and action item follow-up.
• Prepare professional reports, presentations, and briefing materials for internal and external audiences, including Ontario Health.
• Support data collection, compilation, and reporting for project activities.
• Monitor timelines and proactively flag risks or delays to the Project Manager or Senior Director.
• Maintain organized, accessible project documentation using HFHT’s shared platforms.
• Support logistics for patient, community, and stakeholder engagement activities.
Qualifications and Skills:
• Undergraduate degree or diploma in a health-related field, project management, or business administration; equivalent experience considered.
• Minimum 2–3 years of project coordination or administrative support experience, ideally in healthcare or a community health setting.
• Exceptional organizational skills and ability to manage multiple priorities with accuracy and care.
• Proficiency in Microsoft Office and collaboration tools (e.g., SharePoint, MS Teams, Planner); project management software experience an asset.
• Strong written and verbal communication skills and a proactive, professional approach.
• Familiarity with primary care or Ontario Health Team environments is a strong asset.
• Commitment to patient-centered care, equity, and continuous improvement.
