The Outreach, Engagement, and Attachment Worker supports patients in accessing primary care by building strong relationships with healthcare providers, community organizations, and patients. Drawing on their existing relationships and understanding of the health care, social services and other related systems, this individual will collaborate to develop and implement strategies to address barriers to care while aligning efforts with the strategic goals of the Hamilton Family Health Team and the Greater Hamilton Health Network.
Key Responsibilities…
• Collaborating with health and community organizations to identify underserved populations and create pathways for attachment to primary care.
• Engaging with patients, community agencies, and healthcare and act as a liaison and navigator between the patients and providers to ensure clear communication and trust-building.
• Attending and actively participating in community and outreach events to engage with stakeholders and promote healthcare attachment services.
• Engaging with the public to identify healthcare needs and barriers to accessing primary care.
• Providing personalized guidance to patients regarding attachment to primary care; helping them navigate and advocate within healthcare systems and connecting with appropriate primary care providers.
• Address social determinants of health by connecting patients with relevant community resources.
• Assisting in the provision of ethical verbal hand offs/transfers of accountability to facilitate a successful attachment.
• Taking a patient-centred approach when dealing with challenging situations – advocate, intervene and provide patient centered transitions to primary care as required.
• Collecting, understanding and presenting data related to patient attachment rates, primary care availability, and community needs.
• Conducting outreach to community organizations and healthcare providers.
• Facilitating regular communication with community partners to strengthen collaboration.

As a HFHT Dietitian, you will be completing individual nutrition assessments, developing nutrition care plans and providing nutrition counselling for the management of conditions commonly seen in family practice including diabetes, obesity, cardiovascular disease, gastrointestinal disorders, disordered eating, bariatric care and other medical needs. You must have the proven ability to work effectively with patients, physicians and other health professionals in a fast-paced, dynamic team environment. Experience in group facilitation, motivational interviewing and additional languages are considered assets.

Roles and Responsibilities:
- Provide medical nutrition therapy for chronic disease management and prevention identifying nutrition problems, assessing nutritional status of patients, developing nutrition care plans and evaluate and monitoring effectiveness of nutrition interventions
- Conduct comprehensive nutrition assessments identifying stages of change, socio-economic and cultural factors impacting health behaviours
- Utilize appropriate counselling techniques including motivational interviewing and cognitive behavior therapy.
- Communicate nutrition care plans and debrief with primary care team members as needed
- Ensure ongoing nutritional care by maintaining complete and accurate client records, assessment notes and required correspondence, and by monitoring client progress on a regular basis.
- Safeguard client records and assures confidentiality of client information.
- Identify opportunities for quality improvement for nutrition services and interdisciplinary work.
- Work together within RD group to create resources to enhance patient access to timely, evidence-based primary care nutrition.
- Facilitate/co-facilitate groups with other staff of the FHT.
- Assist in the education of staff, students, volunteers and peer support workers and supervision of internship placements.
- Maintain professional competence through participation in professional development activities and maintaining license to practice.
- Comply with all relevant HFHT policies, procedures, and protocols.

Qualifications and Skills:
- 4-year Bachelor of Science or Applied Science degree, majoring in Nutritional Sciences or Human Nutrition (or equivalent); completion of Dietitians of Canada accredited Dietetic Internship program or combined Master’s and Dietetic Internship program
- Good standing with the College of Dietitians of Ontario and possess professional liability insurance.
- Certified Diabetes Educator (CDE) and Second language are an asset
Minimum 2-3 years in a community based or clinical health service organization. - Ability to provide medical nutrition therapy for a wide variety of issues commonly seen in family practice
- A good understanding of nutrition interventions for chronic disease management and evidence-based nutrition best practices

Your Opportunity:
As an OSP CBT Therapist, you will provide individual and group protocol-based structured psychotherapy to individuals experiencing depression, anxiety, and anxiety-related conditions within their community and the Ontario West region. The OSP CBT Therapist will participate in regular clinical consultation to ensure high quality service delivery and adherence to the OSP Clinical Model.
The Hamilton Family Health Team (HFHT) is seeking a temporary (up to 18 months) full-time OSP CBT Therapist 1.0 FTE. The HFHT is actively committed to creating a diverse and inclusive workplace and encourages applications from all qualified candidates.
Roles and Responsibilities:
- Provide individual and group protocol-based structured psychotherapy, including cognitive behavioural therapy (CBT) and cognitive processing therapy (CPT), in alignment with OSP provincial model to individuals experiencing depression, anxiety, and anxiety-related conditions.
- Actively incorporate anti-racist, anti-oppressive practices and principles in the delivery of client-centered, trauma-informed care at all levels of service.
- Work collaboratively with clients, family members, referral sources, community partners, and the OSP program’s network in the provision of accessible, equitable and inclusive care (e.g. interpretation services, culturally appropriate and relevant care, incorporating anti-racism and decolonizing approaches to care, considering social determinants of health and trauma-informed approaches).
- Contribute to ongoing program development and program evaluation and implement new practices as required.
Qualifications and Skills:
- Master’s degree, bachelor’s degree or, an equivalent graduate diploma that requires completion of a bachelor’s level program and has the practice of psychotherapy central to its program.
- Registered member in good standing with one of the five Colleges regulated to perform the Controlled Act of Psychotherapy in Ontario and who are regulated to practice the controlled act of psychotherapy:
-The Ontario College of Social Workers and Social Service Workers
-The College of Nurses of Ontario
-The College of Occupational Therapists of Ontario

- Experience in the provision of mental health and/or addictions treatment (2-3 years preferred), including:

-Documented evidence of formal CBT and CPT training with appropriate supervision/clinical consultation preferred. Additional training in cognitive processing therapy (CPT) preferred
- The science and practice of individual and group-based psychotherapy, Individual and group CBT, CPT and/or other evidence-based psychotherapies for treating clients with depression and/or anxiety (including obsessive-compulsive concerns and post-traumatic stress).
- Distinguishing features of depression, anxiety, and anxiety related conditions, and the etiology and treatment of these disorders.
- Comprehensive understanding of a strengths-based, trauma-informed, client-centered framework for mental health/addictions treatment.
- Motivational interviewing techniques to assess treatment readiness.
- Intersectionality and social determinants of health related to equitable access to, and participation in mental health care.
- Anti-racist, anti-oppressive theory and practice principles in mental health/addictions at a client and system-level.
- Relevant sections of legislation, i.e. Mental Health Act, Personal Health Information Protection Act, Regulated Health Professions Act, Health Care Consent Act, and professional standards to ensure compliance and consistency in provision of client care.
- Computer software to generate correspondence and input data for a variety program specific tools e.g., Microsoft Office Suite, workload measurement, and electronic health records.

The Hamilton Family Health Team (HFHT) is seeking a full-time Facilities Associate 1.0 FTE.
The Facilities Associate is a key frontline role responsible for identifying, troubleshooting, and resolving facilities-related issues that affect patient care and staff operations. Working closely with the Facilities Lead, this position requires strong decision-making and communication skills. This is to ensure efficient coordination of staff requests and workspace modifications in accordance with healthcare standards. The ideal candidate has strong administrative and organizational skills. In addition, the ideal candidate is proactive, responsive, and capable of balancing hands-on tasks with effective coordination and communication across clinical and administrative teams.

Roles and Responsibilities:
- Troubleshoot facilities-related problems—including building and office maintenance, temperature, door access, small appliances, furniture setup, and other items—and determine appropriate next steps, escalating to the Facilities Lead or vendors as needed.
- Communicate effectively with clinical staff, administrative teams, and vendors to understand issues, provide status updates, and ensure timely resolution.
- Coordinate furniture moves and installations, at various hub and practice locations.
- Assist with planning, coordinating and implementing office moves, clinic room setups.
- Support room setups, and room reconfigurations for practitioners and patients, ensuring safety, and workflow needs are met.
- Coordinate room turnovers including tear down, clean up, and set up.
- Communicate with vendors and service providers to facilitate routine maintenance and service needs including cleaners and building management in collaboration with Hub site coordinators.
- Respond and resolve facilities ticket requests via HFHT ticketing system.
- Use sound judgment to prioritize and resolve service requests based on urgency, safety, and impact on patient care and operations.
- Proven ability to make informed decisions through investigative analysis, considering the broader context of patient care—balancing clinical needs, patient safety, ergonomics, and cost-efficiency.
- Implement established health and safety procedures and support compliance efforts.
Conduct monthly Health and Safety Inspections.
Actively support the resolution of Health and Safety concerns addressed by JHSC and concerns brought forward by staff. - Proactively identify and report potential facility hazards or inefficiencies, contributing to continuous improvement efforts.
- Orientation of new HFHT staff – reviewing building security, work location accessibility, and Health and Safety.
- Asset Management – maintain accurate records of furniture and equipment in our asset management system.
- Program and assign security fobs and maintain security door schedules in line with operating hours at each building location.
- Assist in emergency facility repairs with minimal disruption to care delivery.
- Conduct regular inspections of facilities to identify maintenance needs and address them promptly.
- Prepare Standard Operating Procedures and review annually.
- Ability to travel to different HFHT locations and practice sites.

- Health and Safety issues at all HFHT locations.

Qualifications and Skills:
- Completion of post-secondary education; certification/training in facilities management is an asset.
- Minimum of 2 years of experience in administrative/coordination of projects
- Ability to provide quick, effective solutions to emerging issues.
- Demonstrated communication and problem-solving skills.
- Strong troubleshooting skills and ability to assess problems quickly and accurately.
- Confident decision-maker who understands when to act independently and when to escalate.

We are seeking an experienced and dynamic Manager, Primary Healthcare to join our leadership team. This role requires exceptional skills in relationship-building — with physicians, interprofessional staff and providers, community organizations, educational institutions, and non-traditional partners to address the social determinants of health of our community. The Manager co-designs and implements initiatives that improve patient experience, system integration, and population health outcomes for patients interfacing with the HFHT and supporting the broader system across the Greater Hamilton Health Network (GHHN) Primary Care Network.
What You’ll Do
• Strengthen primary health care integration by building trust-based relationships with HFHT physicians, practitioners, and community partners.
• Lead team-based care expansion, co-location initiatives, and other Ontario Health or Primary Care Action Team (PCAT) initiatives, including project planning, documentation, and evaluation.
• Apply quality improvement methods to track progress, capture lessons learned and improve processes.
• Co-design initiatives with patients, providers, and community partners to ensure services are person-centred and equitable.
• Develop, monitor, and manage project budgets to ensure alignment with organizational goals and fiscal accountability.
• Support neighbourhood-based planning efforts, pairing with other managers to ensure consistency, shared learning, and sustainability.
• Providing a primary point of contact for feedback, needs, and problem-solving for the physicians and practices.
• Partner with the Physician Affairs team to ensure physician voices shape planning, policy, and decision-making.

• Minimum 5 years’ progressive leadership experience in healthcare, primary care, integrated healthcare environments, and/or working with physicians.
• Demonstrated experience bridging sectors, leading large organization-wide or system-level initiatives, or system integration.
• Proven relationship-building skills across diverse stakeholders, including physicians, interprofessional teams, and community agencies.
• Strong skills in quality improvement, stakeholder engagement, and collaborative planning.
