Your Opportunity:
We are looking for a highly organized and collaborative Project Coordinator to support the delivery of HFHT’s primary care integration and quality improvement initiatives. Roles and Responsibilities:
• Maintain project plans, schedules, action logs, and status dashboards across PCAT, team-based care expansion, and other strategic initiatives.
• Coordinate meetings and working groups — scheduling, agenda preparation, minute-taking, and action item follow-up.
• Prepare professional reports, presentations, and briefing materials for internal and external audiences, including Ontario Health.
• Support data collection, compilation, and reporting for project activities.

Qualifications and Skills:

Your Opportunity:
We are seeking an experienced and relationship-driven Cyber Security Specialist for a 12-month contract to lead cybersecurity risk assessments and practical risk mitigation planning across a network of affiliated community-based healthcare organizations.
This role works directly with individual physician offices and community partners to assess cybersecurity posture, and identify risks related to health information and operational systems, and develop tailored cybersecurity risk profiles with practical, cost-conscious mitigation strategies.
The ideal candidate combines strong cybersecurity expertise with a collaborative and pragmatic approach. Success in this role requires the ability to build trusted relationships, communicate technical risk in business terms, and develop achievable recommendations that reflect operational realities, organizational maturity, and budget limitations.
The Cyber Security Specialist will work closely with the IT Manager, Privacy Officer, affiliate site leadership, and external vendors and partners as required.
Roles and Responsibilities:
Cybersecurity Risk Assessment & Advisory
· Conduct cybersecurity risk assessments across affiliated community and healthcare sites, including onsite reviews of technical, administrative, and physical safeguards
· Evaluate cybersecurity controls, operational processes, and security practices related to the protection of health information, business systems, and operational continuity
· Assess organizational risk exposure across confidentiality, integrity, and availability domains
· Identify vulnerabilities, control gaps, operational risks, and areas requiring remediation while considering organizational size, technical maturity, and available resources
· Develop site-specific cybersecurity risk profiles, including:
o Risk ratings
o Key findings
o Prioritized recommendations
o Practical remediation opportunities
Risk Mitigation & Practical Planning
· Develop pragmatic, cost-conscious mitigation strategies tailored to smaller healthcare and community organizations with limited internal cybersecurity capacity
· Recommend foundational cybersecurity controls and achievable remediation activities that materially reduce risk without requiring significant capital investment
· Support the development of right-sized cybersecurity roadmaps aligned to operational priorities and risk exposure
· Provide guidance on minimum cybersecurity standards, best practices, and governance expectations appropriate for community healthcare environments
· Identify opportunities for shared services, standardization, and coordinated cybersecurity approaches across affiliated sites where feasible
Stakeholder Engagement & Relationship Management
· Build strong working relationships with leadership, operational teams, technical staff, and external stakeholders across affiliated organizations
· Work collaboratively with organizations operating under independent governance structures and varying levels of cybersecurity maturity
· Understand operational impacts of recommendations, including workflow, staffing, and service delivery considerations
· Act as a trusted cybersecurity advisor by balancing risk reduction with practical operational realities
· Facilitate collaborative discussions to build alignment, trust, and shared accountability across participating organizations and educate on Cybersecurity principals ands best practices in which need to be adapted.
Reporting & Governance
· Prepare clear, concise cybersecurity risk assessment reports and executive summaries for leadership and stakeholders
· Present findings, recurring risk themes, and prioritized recommendations to both technical and non-technical audiences
· Support the development of a consolidated cybersecurity risk view across affiliated organizations to inform strategic planning and prioritization
· Track common risks, trends, remediation activities, and shared mitigation opportunities across the affiliate network
Working Conditions
· Hybrid work model with regular onsite visits to affiliated healthcare and community partner locations

Qualifications and Skills:
Experience:
· 5+ years of progressive experience in cybersecurity, information security, or IT risk management
· Demonstrated experience conducting cybersecurity risk assessments and developing practical remediation plans

Your Opportunity:
As a HFHT Dietitian, you will be completing individual nutrition assessments, developing nutrition care plans and providing nutrition counselling for the management of conditions commonly seen in family practice including diabetes, obesity, cardiovascular disease, gastrointestinal disorders, disordered eating, bariatric care and other medical needs. You must have the proven ability to work effectively with patients, physicians and other health professionals in a fast-paced, dynamic team environment. Experience in group facilitation, motivational interviewing and additional languages are considered assets.
The Hamilton Family Health Team is seeking 1 Registered Dietitian for a 0.6 FTE Part-Time position. The Hamilton Family Health Team is actively committed to creating a diverse and inclusive workplace and encourages applications from all qualified candidates.
Roles and Responsibilities:
- Provide medical nutrition therapy for chronic disease management and prevention identifying nutrition problems, assessing nutritional status of patients, developing nutrition care plans and evaluate and monitoring effectiveness of nutrition interventions
- Conduct comprehensive nutrition assessments identifying stages of change, socio-economic and cultural factors impacting health behaviours
- Utilize appropriate counselling techniques including motivational interviewing and cognitive behavior therapy.
- Communicate nutrition care plans and debrief with primary care team members as needed
- Ensure ongoing nutritional care by maintaining complete and accurate client records, assessment notes and required correspondence, and by monitoring client progress on a regular basis.
- Safeguard client records and assures confidentiality of client information.
- Identify opportunities for quality improvement for nutrition services and interdisciplinary work.
- Work together within RD group to create resources to enhance patient access to timely, evidence-based primary care nutrition.
- Facilitate/co-facilitate groups with other staff of the FHT.

RPN – Attachment Team (1.0 FTE x3)

Your Opportunity:
The Registered Practical Nurse (RPN), Patient Attachment, is a mobile member of HFHT’s Attachment Team, working across all clinic sites to support the attachment of unattached and at-risk patients to primary care providers. This role is central to HFHT’s patient attachment strategy, functioning at the point of care to coordinate outreach, complete clinical onboarding, and facilitate warm connections between patients and their care teams.
The RPN is expected to travel between HFHT clinic sites on a regular basis and may work from various clinical and community settings. Work may include in-person, virtual, and telephone-based interactions. This position is a full-time role and will involve interactions with patients who may be experiencing social vulnerabilities. We are currently looking to hire 3 full time RPNs for HFHT’s Attachment Team.
Roles and Responsibilities:
Patient Outreach and Engagement
• Conduct proactive outreach to unattached patients and those at risk of losing primary care access, via telephone, virtual platforms, or in-person visits.
• Engage with patients in a trauma-informed, culturally responsive manner to support their connection to primary care.
• Facilitate warm handoffs between patients, community partners, and care providers to support seamless attachment at the point of care.
• Identify and address barriers to attachment, including social determinants of health, and connect patients to appropriate resources.
Clinical Onboarding and Chart Preparation
• Initiate patient charts in the electronic medical record (EMR) upon attachment and complete initial patient profiles.
• Gather and document patient medical history, family history, social history, and current medications.
• Review and update immunization records
• Complete screening tools and preventive care checklists as part of the patient onboarding process.
• Prepare clinical summaries to support provider readiness for first patient visits.
Direct Patient Care
• Conduct in-person, virtual, and telephone visits with patients as part of the attachment and onboarding process.
• Perform nursing assessments within the RPN scope of practice, including vital signs, health history collection, and identification of urgent care needs.
• Refer patients to appropriate providers or services when clinical concerns are identified during onboarding interactions.
Community and Partner Engagement
• Build and maintain working relationships with community partners, social service agencies, and health system navigators to enable patient attachment.

The Nurse Practitioner (NP) at the Eva Rothwell Centre is a full-scope primary care provider within Hamilton Family Health Team’s NP-led model at this equity-focused site. The NP holds an enrolled patient panel and is responsible for the full continuum of primary care delivery — from episodic and preventive care through to chronic disease management, care coordination, and patient advocacy — for a population experiencing significant health and social complexity.
The NP practices in accordance with the College of Nurses of Ontario standards for the Extended Class, HFHT policies and procedures, and applicable Ontario legislation.
Roles and Responsibilities:
- Primary Care and Chronic Disease Management
Provide comprehensive, longitudinal primary care to an enrolled patient panel with a high burden of chronic and complex disease - Diagnose, treat, and manage chronic conditions including but not limited to diabetes, hypertension, COPD, heart failure, chronic pain, and mental health conditions
- Develop and implement individualized care plans in collaboration with the patient and interprofessional team

Care Coordination and Navigation
- Lead care coordination for high-complexity patients, facilitating communication across HFHT team members, specialists, and community partners

The Manager, Quality and Patient Experience provides operational leadership for HFHT’s quality improvement, patient experience, and practice sustainability portfolios. Reporting to the Director, Quality, Clinical and Innovation, this role supports the development and implementation of the annual Quality Improvement Plan (QIP), oversees quality data collection and reporting, and leads HFHT’s patient experience and patient safety programs.
A key focus of this role is sustaining and strengthening HFHT’s existing physician members and primary care practices. The Manager works to understand the needs, gaps, and efficiencies within member practices, connects them to organizational supports and core services, and ensures the quality team’s efforts are grounded in the realities of day-to-day primary care. This role recognizes that long-term organizational health depends on the sustainability of the practices at its core.
Roles and Responsibilities:
Quality Improvement
• Supports the Director in developing and implementing the annual Quality Improvement Plan (QIP), including defining indicators, change ideas, and evaluation criteria.
• Leads quarterly data collection, analysis, and reporting processes to track QIP progress and organizational performance.
• Supports the Director in preparing materials and data summaries for the Board Quality Committee.
• Promotes evidence-based practice and the use of QI methodology across HFHT teams and programs.
• Identifies gaps in data availability and works collaboratively to implement solutions.

Practice Sustainability and Efficiency

• Oversees the development and implementation of a practice needs assessment framework, including regular collaboration with Physician Affairs and Primary Healthcare Managers, to engage with physician members to surface gaps, emerging challenges, and opportunities for organizational support.
• Directs the use of EMR and organizational data to build an ongoing picture of practice health, including service utilization, roster status, patient attachment, and clinical performance trends.
• Oversees the assessment and resolution of practice-identified workflow and administrative challenges, leading the development of tailored improvement strategies in partnership with the requesting practice.
• Translates practice-level findings into strategic recommendations for the Director and senior leadership, informing decisions on service development, resource allocation, and organizational priorities.
• Leads the ongoing evolution and scaling of the HFHT Help Centre, an email-based ticketing system that provides physician members and practices with dedicated administrative support in navigating organizational operations and resolving requests in a timely and responsive manner.
• Oversees the analysis of Help Centre data as an organizational intelligence source, using patterns and trends in practice requests to inform priority-setting, service gaps, and strategic recommendations to leadership.
Team Leadership and Management
• Provides day-to-day supervision, coaching, and performance management for Quality Improvement Decision Support Specialists, Practice Facilitators, the Data Quality Coordinator, and the Custom Forms Specialist.
