Primary Care Clinical Pharmacist – 0.4 FTE
Your Opportunity:
As a Primary Care Clinical Pharmacist, you will be part of the team in the fast-paced practices of family physicians. You will work collaboratively with other health professionals to provide comprehensive, accessible primary health care, honouring patient choice in how best care will be delivered (in-person, telephone, video).
Pharmacists in the Hamilton Family Health Team (HFHT) aim to improve drug therapy outcomes using a collaborative care model that integrates pharmacists into the primary care team.
Roles and Responsibilities:
- Integrate into various family medicine group practices by collaborating with physicians, other health care professionals and patients to promote optimal, patient centered care
- Provide individual patient assessments to identify, prevent, and resolve drug-related problems
- Provide pharmaceutical consultation, pharmacotherapy monitoring, education/teaching and drug information

MAiD Coordinator – Temporary Part-time (0.5 FTE)
Your Opportunity:
The Hamilton Family Health Team is one of the largest Ontario Health Teams in the province.
The MAiD Coordinator will have responsibility for coordinating and facilitating MAiD services for patients living in Hamilton, who have requested MAiD support.

Canada that parts of the Criminal Code would need to change to satisfy the Canadian Charter of Rights and Freedoms. The parts that prohibited medical assistance in dying would no longer be valid. The Supreme Court gave the government until June 6, 2016, to create a new law.

Roles and Responsibilities:
- Answering phone calls, providing support to patients and caregivers who may have questions or wish to self-refer.
- Accept referrals, triage patients and liaise with team assessors/providers.
- Maintain a regular caseload, estimated 12-15 open cases at any given time.
- Provide supportive health teaching for patients and caregivers relating to MAiD eligibility criteria and processes.
- Coordinate, attend and participate in patient assessments with team a team including physicians and/or Nurse practitioners.
- Coordinate and assist with all aspects of provision (i.e. organize IV starts, drawing up of meds prn, preparing patient and family etc.)
- Support patients with resources should they wish to donate their organs.

Palliative Care Clinical Coach – TFT
Your Opportunity:
The Hamilton Family Health Team is one of the largest Ontario Health Teams in the province.
The Clinical Coach is responsible for working with participating community organizations in alignment with provincial direction established by Ontario Health and guidance developed by the Ontario Palliative Care Network. The Clinical Coach will drive practice change, with a focus on improving access to and quality of clinical care, aligned to the Palliative Care Health Services Delivery Framework in the Community (“The Delivery Framework”) and the Ontario Palliative Care Competency Framework.
Roles and Responsibilities:
• Provide palliative care coaching and mentoring to staff and clinicians in participating community organizations, to build primary-level palliative care competencies.
• Collaborate with leaders of community organizations on local service integration, and change management initiatives, using quality improvement methodologies to achieve results.
• Consult on clinical care when the complexity of the patients’ needs is beyond the existing competencies of providers (in the community organizations).
• Provide support to Indigenous communities and Indigenous organizations for their implementation efforts, where Indigenous communities wish to be involved, in collaboration with regional partners.
• Co-create the regional Delivery Framework implementation plan with Ontario Health, including required measurement and reporting.
• Attend all regional planning meetings and contribute to the regional palliative care implementation plan, especially plans for education and practice change.
• Provide feedback on key learnings related to coach role, along with providing input and reviewing regional progress reports as required.
• Foster regional collaboration and contribute to broader system integration.
• Develop relationships with key partners to establish trust, credibility, and respect.
• Support Ontario Health Team planning and/or implementation of palliative models of care (where there is readiness).
• Ensure alignment of community implementation with the guidance of the Ontario Palliative Care Network.
• Critically evaluate the quality of palliative care in participating community organizations.
• Lead engagement and community-building activities with participating organizations.
• Build competency amongst front-line community providers in the delivery of primary-level palliative care.
• Offer consultation as a palliative care expert to service providers in person, onsite, by telephone, by videoconference or through email.
• Provide support to Indigenous communities and organizations for their implementation efforts, as requested by those organizations and, where appropriate, in collaboration with regional partners
• Guide the creation of a quality improvement plan to address gaps.
• Provide limited and occasional direct patient care.

- Post secondary degree in a registered health care discipline (e.g., RN, RSW, NP) with substantive experience and training in palliative care (e.g., CAPCE). A graduate degree is an asset (e.g., MScN, MSW).
- Minimum five (5) years’ experience in practice in palliative care is required.
- Knowledge of relevant legislation and reporting requirements, including the Fixing Long-Term Care Homes Act, the Excellent Care for All Act and the Compassionate Care Act and other pertinent legislation and regulations related to professional practice is required.
- Experience supervising clinical staff is preferred.
- Experience working with large-scale quality improvement initiatives.
- Additional training in gerontology and project management experience are assets.
- Strong clinical coaching and debriefing skills, with both individuals and teams.
- Strong consultation skills, with expert knowledge of models of consultation and their application with individual, team and community service providers.
- Research and/or program evaluation experience with knowledge of evidence-based processes, including appraisal of research, application of findings and collaboration on research.
- Experience working with a range of roles, disciplines, and levels of staff in different health care settings.
- Experience working across health care settings.
- Fluency in French and/or another language is an asset.

Registered Dietitian
Your Opportunity:
As a HFHT Dietitian, you will be completing individual nutrition assessments, developing nutrition care plans and providing nutrition counselling for the management of conditions commonly seen in family practice including diabetes, obesity, cardiovascular disease, gastrointestinal disorders, disordered eating, bariatric care and other medical needs. You must have the proven ability to work effectively with patients, physicians and other health professionals in a fast-paced, dynamic team environment. Experience in group facilitation, motivational interviewing and additional languages are considered assets.

Mental Health RN
Your Opportunity:
As a HFHT Mental Health RN (Intake & Triage Role), you will be completing individual assessments, developing care plans and increasing access to direct and indirect consultation for HFHT patients whose family physicians do not have on-site psychiatry consultation. Solo physician seeking a dedicated RN to join our healthcare team-based interdisciplinary medical practice in Central Hamilton. Days of work will be Monday, Tuesday, Thursday, and Friday.
Job Summary
Working under the direction of the physician, you will play a crucial role in providing high-quality patient care.
Duties:
Provide direct patient care in our family practice clinic with a focus on compassion and empathy
Administer medications and treatments as prescribed by the physician
Monitor patient vital signs and symptoms, reporting any changes promptly
Educate patients and their families on health management and disease prevention strategies
Utilize medical terminology accurately in documentation and communication
Handle acute care situations with professionalism and efficiency
Coordinate with interdisciplinary teams to deliver optimal patient outcomes
Utilize electronic health record systems effectively
Experience
Bachelor’s degree in nursing or equivalent
Valid registration with the Ontario Nursing Association
