As a Hub Site Coordinator, will provide comprehensive administrative and logistic support to our healthcare professionals in a team setting.
The Hamilton Family Health Team (HFHT) is seeking a full-time Hub Site Coordinator 1.0 FTE.

Roles and Responsibilities:
- Ensuring the efficient and smooth operation of HFHT Hub site/office,
- Managing administrative tasks; overseeing the overall flow of the office environment
- Booking patient appointments, greeting patients, and answering the HFHT phone line
- Liaising with patients and families in a professional and compassionate manner while maintaining privacy and confidentiality
- Maintaining patient records by obtaining, recording, and updating personal information
- Scanning and faxing patient information and consultation reports
- Liaising with practice–based primary care teams
- Overseeing and monitoring room bookings and scheduling
- Other duties as required
Qualifications and Skills:
- BA or diploma in Medical Office Administration or equivalent
- 3-5 Years of Medical Administrative Experience
- Exceptional customer service, communication and organizational skills
- Excellent interpersonal skills, with proven ability to work well in a team environment
- Demonstrated problem solving skills and initiative
- Proficient with Microsoft Office
- Must be able to multi-task and prioritize while working within a busy environment
- Experience with EMR (Practice Solutions Suite) an asset
- Previous experience in a clinical setting an asset
