Increasing Access to Primary Care in our Local Community

From April 2024 to March 2025, we significantly expanded our health education groups & workshops, offering new and existing workshops to the broader Hamilton community free of charge, working with our Primary Care Network partners through funding from Ontario Health. We also provided patients who previously would not have access to team-based primary care with a suite of care options through TeamCare+.

From April 1, 2024 to March 31, 2025:

patients supported through TeamCare+, which has been rolled out to 7 unaffiliated physicians
