Celebrating Pride Month: Affirming Care at HFHT

At Hamilton Family Health Team (HFHT), we recognize the importance of creating safe, affirming spaces for 2SLGBTQIA+ communities—not just during Pride Month, but all year long.

In honour of Pride, we’re highlighting two free programs: AFFIRM and our Gender-Affirming Care workshop. Both offerings reflect HFHT’s ongoing commitment to equity, accessibility, and patient-centred care.

1. AFFIRM

AFFIRM is an eight-session, in-person group designed for 2SLGBTQIA+ youth aged 11–18, providing a supportive space to explore identity, manage emotions, and build confidence. Originally launched through the University of Toronto’s Project Youth AFFIRM, HFHT was one of the host agencies trained to deliver the program.

Through collaborations with the Hamilton-Wentworth District School Board and Compass Community Health, the program has expanded to serve youth across the community.

We know that for 2SLGBTQIA+ youth community can be lifesaving, and this group supports folks to build and expand their networks of support and meet more people who have shared experiences and identities.

Participants learn to:

  • Strengthen positive thinking
  • Reduce depression and anxiety
  • Improve coping and decision-making skills
  • Celebrate personal strengths

Groups run after school for youth ages 11–13 and 14–18.

2. Gender-Affirming Care Workshop

HFHT’s Gender-Affirming Care workshop is designed for transgender and gender-diverse individuals considering or currently undergoing hormone therapy. This interactive session focuses on education, empowerment, and personalized care.

Participants can expect to learn about:

  • Types of hormone therapy (testosterone, estrogen, anti-androgens)
  • What changes to expect and how to monitor them
  • Mental health support, fertility planning, and long-term wellness
  • Frequently asked questions:
    • How fast will I see changes?
    • Can hormone therapy affect my mood?
    • Is it reversible?
    • What lifestyle habits help support treatment?

This session is delivered in collaboration with Queen Square Family Health Team and West Durham Family Health Team, combining resources and expertise to ensure broad, compassionate care.

Participants leave with:

  • A strong understanding of hormone therapy options and monitoring
  • Tips for navigating physical changes and staying healthy
  • Clear info on where to turn for support
