The People of Primary Care
Better data, better care with Maryam Ojo
In recognition of Healthcare Quality Week we’d like to introduce some of Hamilton Family Health Team’s Quality Improvement Team!
Please Introduce Yourself
I’m Maryam Ojo, Quality Improvement Decision Support Specialist
What Do You Do?
A big part of what I do is translate complex health and EMR (Electronic Medical Record) data into meaningful insights that support quality improvement initiatives, identify opportunities for clinical process change, and inform decision-making. I partner with teams to develop improvement plans, strengthen data quality and reporting practices, and build their confidence in using performance measures to drive change.

What Brought You To This Profession?
I love working in roles that allow me be part of transformational change, especially when it leads to population health improvement. This role attracted me because it provides a unique opportunity to improve how patients experience the health system by supporting the physicians and optimizing the way they practice.
Why Primary Care?
Primary care is the foundation of our health system, beyond public health, it is how the general population is cared for at the basic level before complications could arise. I also love the emphasis on team-based care as it creates an ideal wrap-around care model for patients. I am glad to be part of such impactful work.
What Brings You Joy in this Role?
It’s enjoy getting to work with different physicians/practices, getting to understand their challenges and leveraging resources and tools to be able to help make their work easier and more efficient.
Where can we find you when you’re not in the office?
I love to travel and also visit museums. This photo of me is at the Painted Hall in Greenwich, during a trip to the UK!

Thank you Maryam and the rest of the Q.I. team! Your contributions, willingness to go over and above, and can-do spirit drives the ongoing support received by many across the organization. Your hard work truly makes a difference, and we appreciate all that you do
