We’re a team of health care providers and support staff working closely with you to provide comprehensive, team-based primary health care to residents of the Greater Hamilton Area.
Founded by the Ontario government in 2005, Family Health Teams are community- and patient-centred organisations that provide primary health care programs and services tailored to the residents of their area. The Hamilton Family Health Team is the largest Family Health Team in Ontario, serving nearly half of Hamilton’s population.
We help keep our patients healthy, treat them when they’re sick, and provide health education. You might see us in your doctor’s office, at a health care hub, in a virtual workshop, or at an event in your community. We are:

What Else We Do
- Provide practice support in the form of tech support, HR support, patient health privacy inquiries, practice quality improvement and digital resources.
- Partner with local organizations to run advocacy and health-based events and programs.
- Have a Green Initiative program that advocates and helps primary care clinics be more environmentally conscious.
- Provide complaint resolution for patients, and try and build a better patient experience.
- Have an attachment pipeline to connect patients to family doctors

Your health matters.
We want a future where everyone in Hamilton has access to the care they need, where and when they need it. “Primary care homes” will integrate primary health care with social supports and community belonging – we are working with our Primary Care Network partners, the Ontario government, and community partners to make this vision a reality. Check out our newest primary care clinic, Health Care at Eva Rothwell Centre, as an example of what we’re working towards.

Our Mission, Vision and Strategic Plan
A healthy community is a community where all individuals have access to primary care. Our Strategic Plan is about a partnership amongst our patients, our providers, and our partners. Our Plan will help to improve health outcomes and will optimize the use of valuable health care resources by addressing gaps in services and focusing on preventative care while creating a meaningful and effective health care experience for patients and providers alike. Our Strategic Plan will help us to elevate primary care in Hamilton. We will reach new heights together.
