18-Month Maternity Leave Contract

Location: Southwest Family Health Centre
Status: 3 – 5 days a week, 18-Month Contract
Hours: 9 – 5
Compensation: $36.00 – $38.00/hour depending on experience.
Start Date: September 8, 2026

Southwest Family Health Centre is seeking an experienced Registered Nurse (RN) to join our primary care team for an 18-month maternity leave contract.

Responsibilities

• Provide nursing care, assessment, triage and patient follow-up.
• Support chronic disease management, preventative care and health promotion.
• Administer injections, immunizations and provide wound care as required.
• Assist with procedures, referrals, investigations and results follow-up.
• Provide patient education and telephone support.
• Maintain accurate and confidential electronic medical records.
• Collaborate with physicians, nurse practitioners and other healthcare professionals.

Requirements

• Current registration with the College of Nurses of Ontario (CNO) and in good standing.
• Primary care/family practice experience required.
• Experience with Practice Solutions Software (PSS) required.
• Strong clinical assessment, communication and organizational skills.
• Ability to work independently and as part of a team.
• Commitment to patient-centred care and confidentiality.
