Your Opportunity:
The Hamilton Family Health Team is seeking applicants for one full-time Registered Practical Nurses at the Healthcare at Eva Rothwell Centre located at the Eva Rothwell Centre, 460 Wentworth Street North, a trusted community facility since 2004, where social and community programs complement the activities of the clinic.
Duties and Responsibilities:
- Chronic Disease Management Support
Assist in the monitoring and follow-up of patients with chronic conditions (e.g., diabetes, hypertension, COPD, heart failure) under the direction of the NP - Perform clinical tasks including vital signs, point-of-care testing, wound care, and specimen collection
- Provide patient education on disease management, medications, and self-care strategies
- Support recall and follow-up processes for patients with complex chronic disease needs
Preventive Care, Well-Woman, Well-Baby, and Prenatal Care
- Support well-woman visits including routine health screening, Pap tests (as per scope and HFHT protocols), and reproductive health education
- Assist with well-baby and well-child visits including growth monitoring, developmental screening, and caregiver education
- Provide prenatal support including blood pressure monitoring, urinalysis, fundal height measurement, fetal heart rate auscultation, and prenatal education
- Administer vaccines according to Ontario immunization schedules and HFHT medical directives, including documentation and cold chain management
- Conduct immunization counselling and address vaccine hesitancy using evidence-informed approaches
- Support recall and reminder processes for overdue immunizations and preventive screening
Care Coordination and Navigation
- Assist patients in navigating health and social services, including referrals to specialist care, community agencies, and HFHT allied health providers
- Support warm handoffs between team members, including social work, dietetics, and community health workers
- Participate in care planning for high-complexity patients alongside the NP and interprofessional team
- Liaise with the Eva Rothwell Centre’s community programs to support integrated care
Clinical Administration and Triage
- Conduct telephone and in-person triage, assess patient needs, and prioritize care in collaboration with the NP
- Manage patient flow and prepare patients for clinical encounters
- Follow-up on patient’s blood work and imaging results as per NP direction
- Maintain accurate, timely EMR documentation in accordance with CNO standards and HFHT policies
- Support medication management processes including prescription preparation and reconciliation tasks within RPN scope
- Contribute to quality improvement activities, including data collection and chart audits
- Responsible for ordering vaccines and clinical supplies, stocking clinical rooms, and sterilizing clinical tools
Qualifications and Skills:
- Current registration in good standing with the College of Nurses of Ontario as a Registered Practical Nurse
- Completion of a recognized RPN diploma or nursing program
- Current Basic Life Support (BLS/CPR) certification
- Demonstrated commitment to equity-centered, trauma-informed, and culturally safe care
- Strong interpersonal communication skills with the ability to build trust with marginalized populations
- Experience with electronic medical records (Telus PS preferred) considered an asset
