Family Health Teams (FHTs) are a key piece of the Ontario government's plan to build a health care system that promotes good health, improves the care individuals receive, reduces wait times and increases access to care.

In Family Health Teams, family doctors work in teams with other health care professionals to see more patients and to keep them healthy.

The Hamilton Family Health Team (HFHT) includes 129 family doctors, nurses and nurse practitioners, dietitians, mental health counsellors, psychiatrists and pharmacists.  These teams are located in many different locations across the city of Hamilton, and together they serve over 250,000 people.

Programs

Mental Health - The HFHT Mental Health Program brings mental health services into the offices of participating family doctors in Hamilton.

Nutrition Program - The goal of the HFHT Nutrition Program is to expand the range of nutrition services available in family doctors' offices

Pharmacy Program - The HFHT Pharmacy Program brings clinical pharmacists into the offices of participating family doctors in Hamilton, Ontario.

Nursing Program - RN's within the HFHT work with our doctors, mental health counselors, dietitians and pharmacists to provide primary care that promotes health and prevents disease.


Projects & Pilots

Peer Support - Our goal is to connect people living with depression with a trained Peer Support Worker (someone who has experienced living with depression).  Peer Support Workers provide additional support, but do not replace the care provided by the medical team.  [more]

Substance Use - Increase the capacity of primary care settings to effectively detect and treat patients experiencing problematic substance use.

Child & Youth Mental Health - Promote healthy development of all children by raising awareness and increasing detection and access to Child and Youth Mental Health. 

Enhanced Depression Care Initiative - Our goal is to enhance assessment, treatment, and follow-up of patients with depression using a team approach.
