Step 1: Make sure you’re signed up for Health Care Connect*.

Step 2: Reach out to our friendly Patient Attachment Team. We’ll help connect you to a doctor who’s accepting patients. Call us at 365-317-8117, or email attach@hamiltonfht.ca. Due to the volume of patients we are helping, it may take 4-6 weeks to receive a response.
