HFHT Patient Interpretation and Translation Services

Steps

1. View the GHHN FAQ document with instructions on how to use this service. 
2. Access the forms and instructions for importing the forms into the EMR, from the HFAM website here:  https://hfam.ca/clinical-pathways-and-evidence/emr-tools/. You could also download the pdf form here
3. Submit the ITS Interpretation Request Form to buckmilc@mcmaster.ca. Please title the email: GHHN PCN ITS Request. McMaster Admin will then connect with ITS and ITS will confirm an interpreter with your clinic. Do not send the completed form directly to ITS.

Be sure to include your practice/ doctor name, and the kind of EMR you are using, so as to have your question directed to the appropriate party.
