Patient and Family Advisory Group
In 2019, the HFHT made a commitment to form a Patient and Family Advisory Group (PFG) to make sure that the voices of patients and families are heard, considered and included in patient care. The group gives patients an opportunity to share their experiences, insights and opinions with a goal of improving care to better meet the needs of patients and families (Patient-Centred Care).
Practice Highlight: Core Care Family Health Team
The Hamilton Family Health Team partners with over 160 family physicians at more than 90 practice sites across the Greater Hamilton Area. One of our practice sites, Core Care Family Health Team, speaks to their committment to patient-centred care as their two founding physicians enter into retirement.

Dr. Jean Mullens and Dr. Brenda Copps founded their family medicine practice in 1986, shortly after reconnecting following medical school. United by a shared passion for helping others and improving access to care, they built a clinic rooted in compassion, collaboration, and a deep commitment to the Hamilton community. As early champions of team-based care, they helped pave the way for what would become the Core Care Family Health Team—now home to four family physicians, two physician assistants, and a dedicated team of ten staff. As proud members of the Hamilton Family Health Team, we continue to provide high-quality, patient-centered care that reflects the values they’ve lived by for over 40 years. While their upcoming retirement will leave a meaningful gap, we remain committed to carrying their vision forward and continuing to serve our community with the same heart and dedication.
