In March of 2024, our joint funding proposal with primary care providers across the Greater Hamilton Health Network (GHHN) was successful, receiving $2.2 million from Ontario Health to expand primary care teams across the GHHN.
Over the last year, in partnership with the Greater Hamilton Health Network Primary Care Network (GHHN PCN), we have connected over 15,500 people across Hamilton, Haldimand, and Niagara Northwest to team-based primary care!
