Data Quality Coordinator

Your Opportunity:

We are currently recruiting for a full-time Data Quality Coordinator (1.0 FTE) to join our team.

The Data Quality Coordinator will play a critical role in ensuring accurate and timely reporting to Ontario Health and the Primary Care Stakeholder Council. This position involves coordinating reporting requirements, supporting the building of efficient electronic medical records (EMR) queries for data collection, and conducting regular audits to maintain data accuracy and integrity.

A Successful Candidate in This Role Will Have:

  • Bachelor’s degree in Health Informatics, Data Management, Information Technology or a related field
  • Minimum 2 years experience in data management, reporting, or a related role in a healthcare setting; experience with primary care data is preferred
  • Proficiency in electronic medical record systems, data querying and reporting tools. Experience with Excel.

Duties and Responsibilities:

  • Support Quality Improvement Decision Support Specialists (QIDSS) to collaborate with physician and practice teams to build electronic medical record (EMR) queries to collect measurement information
  • Support the preparation and presentation of detailed reports for regular meetings with partners and Ontario Health

Registered Dietitians (x2 positions) – part-time (0.6 FTE) / full-time (1.0 FTE)

Your Opportunity:

As a HFHT Dietitian, you will be completing individual nutrition assessments, developing nutrition care plans and providing nutrition counselling for the management of conditions commonly seen in family practice including diabetes, obesity, cardiovascular disease, gastrointestinal disorders, disordered eating, bariatric care and other medical needs. You must have the proven ability to work effectively with patients, physicians and other health professionals in a fast-paced, dynamic team environment. Experience in group facilitation, motivational interviewing and additional languages are considered assets.

The Hamilton Family Health Team is seeking two Registered Dietitians for a 0.6 FTE (24 hours per week) and a full-time 1.0 FTE (40 hours per week) position. Roles and Responsibilities:

• Provide medical nutrition therapy for chronic disease management and prevention identifying nutrition problems, assessing nutritional status of patients, developing nutrition care plans and evaluate and monitoring effectiveness of nutrition interventions
• Conduct comprehensive nutrition assessments identifying stages of change, socio-economic and cultural factors impacting health behaviours
• Utilize appropriate counselling techniques including motivational interviewing and cognitive behavior therapy.
• Communicate nutrition care plans and debrief with primary care team members as needed
• Ensure ongoing nutritional care by maintaining complete and accurate client records, assessment notes and required correspondence, and by monitoring client progress on a regular basis.
• Safeguard client records and assures confidentiality of client information.
• Identify opportunities for quality improvement for nutrition services and interdisciplinary work.
• Work together within RD group to create resources to enhance patient access to timely, evidence-based primary care nutrition.
• Facilitate/co-facilitate groups with other staff of the FHT.
• Assist in the education of staff, students, volunteers and peer support workers and supervision of internship placements.
• Maintain professional competence through participation in professional development activities and maintaining license to practice.

Social Worker – 0.6 FTE

We are currently recruiting for a regular part-time Social Worker (0.6 FTE) to join our Interprofessional Primary Care team.

The Primary Care Social Worker is part of an interprofessional health team, providing biopsychosocial assessments and interventions for diverse populations. Primary Care Clinical Pharmacist – 0.3 FTE

Palliative Care Clinical Coach – TFT

Your Opportunity:

The Hamilton Family Health Team is one of the largest Ontario Health Teams in the province.

The Clinical Coach is responsible for working with participating community organizations in alignment with provincial direction established by Ontario Health and guidance developed by the Ontario Palliative Care Network. The Clinical Coach will drive practice change, with a focus on improving access to and quality of clinical care, aligned to the Palliative Care Health Services Delivery Framework in the Community (“The Delivery Framework”) and the Ontario Palliative Care Competency Framework.

Roles and Responsibilities:

• Provide palliative care coaching and mentoring to staff and clinicians in participating community organizations, to build primary-level palliative care competencies.
• Collaborate with leaders of community organizations on local service integration, and change management initiatives, using quality improvement methodologies to achieve results.
• Consult on clinical care when the complexity of the patients’ needs is beyond the existing competencies of providers (in the community organizations).
• Provide support to Indigenous communities and Indigenous organizations for their implementation efforts, where Indigenous communities wish to be involved, in collaboration with regional partners.
• Co-create the regional Delivery Framework implementation plan with Ontario Health, including required measurement and reporting.
• Attend all regional planning meetings and contribute to the regional palliative care implementation plan, especially plans for education and practice change.
• Provide feedback on key learnings related to coach role, along with providing input and reviewing regional progress reports as required.
• Foster regional collaboration and contribute to broader system integration.
• Develop relationships with key partners to establish trust, credibility, and respect.
• Support Ontario Health Team planning and/or implementation of palliative models of care (where there is readiness).
• Ensure alignment of community implementation with the guidance of the Ontario Palliative Care Network.
• Critically evaluate the quality of palliative care in participating community organizations.
• Lead engagement and community-building activities with participating organizations.
• Build competency amongst front-line community providers in the delivery of primary-level palliative care.
• Offer consultation as a palliative care expert to service providers in person, onsite, by telephone, by videoconference or through email.
• Provide support to Indigenous communities and organizations for their implementation efforts, as requested by those organizations and, where appropriate, in collaboration with regional partners
• Guide the creation of a quality improvement plan to address gaps.
• Provide limited and occasional direct patient care.


If so, we are looking for a future colleague who has…

  • Post secondary degree in a registered health care discipline (e.g., RN, RSW, NP) with substantive experience and training in palliative care (e.g., CAPCE). A graduate degree is an asset (e.g., MScN, MSW).
  • Minimum five (5) years’ experience in practice in palliative care is required.
  • Knowledge of relevant legislation and reporting requirements, including the Fixing Long-Term Care Homes Act, the Excellent Care for All Act and the Compassionate Care Act and other pertinent legislation and regulations related to professional practice is required.
  • Experience supervising clinical staff is preferred.
  • Experience working with large-scale quality improvement initiatives.
  • Additional training in gerontology and project management experience are assets.
  • Strong clinical coaching and debriefing skills, with both individuals and teams.
  • Strong consultation skills, with expert knowledge of models of consultation and their application with individual, team and community service providers.
  • Research and/or program evaluation experience with knowledge of evidence-based processes, including appraisal of research, application of findings and collaboration on research.
  • Experience working with a range of roles, disciplines, and levels of staff in different health care settings.
  • Experience working across health care settings.
  • Fluency in French and/or another language is an asset.
  • Ability to work well under pressure and use good judgment in assessing difficult situations.
  • Ability to work independently and as part of a team.
  • Experience designing education to effectively address palliative care training needs of interprofessional learners.

Executive Lead, Integrated Mental Health and Addictions Crisis and Outreach Response (GHHN)

Location: Hamilton, Ontario

Overview:
The Greater Hamilton Health Network (GHHN), in collaboration with the City of Hamilton, is seeking a highly motivated and experienced individual to lead a transformative project aimed at improving the coordination, integration, and delivery of mental health and addictions (MHA) crisis and outreach response across the City of Hamilton. This role will involve overseeing the development and implementation of an integrated service model, ensuring seamless collaboration between various healthcare providers, social services, and community outreach programs to enhance accessibility, care, and responsiveness to clients in crisis.
The senior leader will be seconded to or employed by the GHHN with oversight from both the City of Hamilton and GHHN, and will require effective leadership, project management, and stakeholder engagement to achieve the objectives. While full time temporary senior leader is sought for this initiative, consideration will be provided to secondment of a part time leader, with team support.
Key Responsibilities:

Project Leadership and Management:
o Lead the development, planning, and execution of a comprehensive work plan for integrating mental health and addictions crisis and outreach response across Hamilton.
o Manage day-to-day project activities, ensuring that deliverables are achieved according to timelines and within scope.

Coordination of Stakeholder Engagement:
o Facilitate collaboration between a diverse range of stakeholders, including healthcare providers, social service agencies, municipal partners, individuals with lived experience, and the public.
o Champion inclusive consultation processes to ensure that the integrated response model reflects the diverse needs of the community.

Strategic Assessment and Feasibility Studies:
o Lead the mapping and inventory of Hamilton’s mental health and addictions sector, identifying current services, gaps, redundancies, and opportunities for improved coordination.
o Assess existing crisis and outreach response in Hamilton, including evaluating effectiveness, service delivery, funding streams, and client populations served.
o Investigate the feasibility of a 211 response system for Hamilton, for ensuring both a non-police response for appropriate calls and mobilization of available community supports, including full costing, phased implementation, and exploration of alternative single point of access coordinated response models that better meet local needs. Development of Integrated Service Model:
o Develop actionable recommendations for an integrated service delivery model, including resource allocation, crisis de-escalation, and follow-up care.

Reporting and Accountability:
o Provide regular updates to both City of Hamilton and GHHN leadership teams, including City Council and the GHHN Board of Directors.
Qualifications:
o A relevant degree in health administration, public health, social work, psychology, mental health, or a related field. A Master’s degree is an asset.
o Minimum 5-7 years of experience in progressive leadership roles, particularly in the health, social services, or mental health sector.

o Experience in leading cross-functional teams and managing complex, multi-stakeholder projects.
o Proven track record in developing and implementing integrated service models or systemic change initiatives.
o Knowledge of Ontario’s healthcare and social service landscape; local knowledge of Hamilton’s system is an asset.
o Strong leadership and team management skills, with the ability to foster collaboration and drive results.
o Excellent communication, presentation, and interpersonal skills, with the ability to engage diverse stakeholders effectively.
o Analytical and strategic thinking, with the ability to assess complex situations and propose practical solutions.
o Demonstrated ability to work with government, community organizations, and individuals with lived experience.
o Capacity to develop and manage budgets, timelines, and resources effectively.

Additional Information:
This position will require regular travel within the City of Hamilton and coordination with various stakeholders across the region. To balance the need for a senior leader to hold this position, and the temporary nature of the role, the successful candidate can either be seconded from a host organization to the GHHN, or directly employed, depending on individual circumstance. The role will report to both the City of Hamilton and GHHN leadership teams. It is expected that this will be a multi-year project, with phased implementation of recommendations spanning two years or more.
