Your Opportunity:

The Hamilton Family Health Team (HFHT) is seeking a full-time Facilities Associate 1.0 FTE.

The Facilities Associate is a key frontline role responsible for identifying, troubleshooting, and resolving facilities-related issues that affect patient care and staff operations. Working closely with the Facilities Lead, this position requires strong decision-making and communication skills to ensure that all requests, repairs, and workspace modifications are handled efficiently and in accordance with healthcare standards. The ideal candidate is independent, proactive, responsive, and capable of balancing hands-on tasks with effective coordination and communication across clinical and administrative teams.

Roles and Responsibilities:
• Troubleshoot facilities-related problems—including building and office maintenance, temperature, door access, lighting, HVAC, plumbing, small appliances, furniture setup, and other items—and determine appropriate next steps, escalating to the Facilities Lead or vendors as needed.
• Communicate effectively with clinical staff, administrative teams, and vendors to understand issues, provide status updates, and ensure timely resolution.
• Coordinate furniture moves and installations, at various hub and practice locations.
• Assist with planning, coordinating and implementing office moves, clinic room setups.
• Support room setups, and room reconfigurations for practitioners and patients, ensuring safety, and workflow needs are met.
• Coordinate room turnovers including tear down, clean up, and set up.

• Respond and resolve facilities ticket requests via HFHT ticketing system.
• Use sound judgment to prioritize and resolve service requests based on urgency, safety, and impact on patient care and operations.
• Proven ability to make informed decisions through investigative analysis, considering the broader context of patient care—balancing clinical needs, patient safety, ergonomics, and cost-efficiency.
• Implement established health and safety procedures and support compliance efforts.
• Conduct monthly Health and Safety Inspections.
• Actively support the resolution of Health and Safety concerns addressed by JHSC and concerns brought forward by staff.
• Proactively identify and report potential facility hazards or inefficiencies, contributing to continuous improvement efforts.
• Orientation of new HFHT staff – reviewing building security, work location accessibility, and Health and Safety.
• Asset Management – maintain accurate records of furniture and equipment in our asset management system.
• Program and assign security fobs and maintain security door schedules in line with operating hours at each building location.
• Assist in emergency facility repairs with minimal disruption to care delivery.
• Conduct regular inspections of facilities to identify maintenance needs and address them promptly.
• Prepare Standard Operating Procedures and review annually.
• Ability to travel to different HFHT locations and practice sites.
• Key contact for any building maintenance and Health and Safety issues at all HFHT locations.

Qualifications and Skills:
• Completion of post-secondary education; certification/training in facilities management is an asset.
• Minimum of 2 years of experience in facility support, maintenance, or coordination.
• Working knowledge of building systems.
• Ability to provide quick, effective solutions to emerging issues.
• Demonstrated communication and problem-solving skills.
• Strong troubleshooting skills and ability to assess problems quickly and accurately.
• Confident decision-maker who understands when to act independently and when to escalate.

Your Opportunity:

We are currently recruiting for a full-time Triage Clinician (1.0 FTE) to join our Interprofessional Primary Care team.

The Triage and Float Clinician is responsible for assessing and prioritizing patient needs within the primary care centralized hub(s). This role ensures timely access to appropriate care by evaluating symptoms and functioning, assessing social determinants of health, determining urgency, and directing patients to the most suitable healthcare services. The clinician will collaborate closely with primary care providers, interdisciplinary healthcare providers and community resources to ensure seamless patient care.

A Successful Candidate in This Role Will Have:

  • Post secondary degree/education at the Bachelors or Master’s level education in Nursing, Social Work or other healthcare related field.
  • Registration with the related college in good standing required.
  • Minimum of 5 years’ experience in clinical practice, preferably in primary care, emergency care, or mental health setting.

Duties and Responsibilities:

  • Patient Assessment & Triage: Conduct brief initial patient assessments via phone, virtual consultations, or in-person to determine the level of urgency and appropriate care pathway.
  • Clinical Decision-Making: Utilize clinical judgment and evidence-based guidelines to triage patients to primary care providers, secondary care, tertiary care, community services, emergency services, interdisciplinary team-based care services or other specialized care and programs.
  • Care Coordination: Work closely with HFHT physicians, NPs, PAs, nurses, mental health counselors, social workers, registered dietitians, pharmacists, physiotherapists and psychiatrists while also coordinating with our community and health care partners to facilitate timely referrals and follow-up care.
  • Brief Intervention and Case Management: Provide brief, solution-focused interventions and carry a small patient caseload as patients are connected to the appropriate resources.
  • Crisis Intervention: Identify and manage urgent cases, providing de-escalation support and appropriate crisis referrals.
  • Patient Education: Provide patients and families with guidance on symptom management, self-care strategies, and when to seek further medical attention.
  • Documentation & Compliance: Maintain accurate and timely documentation in electronic medical records (EMRs), adhering to regulatory and organizational standards.
  • Quality Improvement: Participate in ongoing training, case reviews, track and manage data and process improvement initiatives to enhance triage efficiency and patient care outcomes.
  • Float: May be required to carry a small patient case load or back-up mental health coverage if capacity allows.

Your Opportunity:

As an HFHT Dietitian, you will be completing individual nutrition assessments, developing nutrition care plans and providing nutrition counselling for the management of conditions commonly seen in family practice including diabetes, obesity, cardiovascular disease, gastrointestinal disorders, disordered eating, bariatric care and other medical needs.  You must have the proven ability to work effectively with patients, physicians and other health professionals in a fast-paced, dynamic team environment.  Experience in group facilitation, motivational interviewing and additional languages are considered assets.  

The Hamilton Family Health Team is seeking a Registered Dietitian for a temporary full-time (1.0 FTE) position, for a period of up to 12 months. Provide medical nutrition therapy for chronic disease management and prevention identifying nutrition problems, assessing nutritional status of patients, developing nutrition care plans and evaluate and monitoring effectiveness of nutrition interventions
·         Conduct comprehensive nutrition assessments identifying stages of change, socio-economic and cultural factors impacting health behaviours

·         Utilize appropriate counselling techniques including motivational interviewing and cognitive behavior therapy. 

·         Communicate nutrition care plans and debrief with primary care team members as needed

·         Ensure ongoing nutritional care by maintaining complete and accurate client records, assessment notes and required correspondence, and by monitoring client progress on a regular basis.

·         Safeguard client records and assures confidentiality of client information.

·         Identify opportunities for quality improvement for nutrition services and interdisciplinary work.

·         Work together within RD group to create resources to enhance patient access to timely, evidence-based primary care nutrition.

·         Facilitate/co-facilitate groups with other staff of the FHT.

·         Assist in the education of staff, students, volunteers and peer support workers and supervision of internship placements. 

·         Maintain professional competence through participation in professional development activities and maintaining license to practice.

·         Comply with all relevant HFHT policies, procedures, and protocols.

Qualifications and Skills:

 

·         4-year Bachelor of Science or Applied Science degree, majoring in Nutritional Sciences or Human Nutrition (or equivalent); completion of Dietitians of Canada accredited Dietetic Internship program or combined Master’s and Dietetic Internship program

·         Good standing with the College of Dietitians of Ontario and possess professional liability insurance.

Certified Diabetes Educator (CDE) and Second language are an asset
·         Minimum 2-3 years in a community based or clinical health service organization.

·         Ability to provide medical nutrition therapy for a wide variety of issues commonly seen in family practice

Your Opportunity:

As a HFHT Dietitian, you will be completing individual nutrition assessments, developing nutrition care plans and providing nutrition counselling for the management of conditions commonly seen in family practice including diabetes, obesity, cardiovascular disease, gastrointestinal disorders, disordered eating, bariatric care and other medical needs. Roles and Responsibilities:

  • Provide medical nutrition therapy for chronic disease management and prevention identifying nutrition problems, assessing nutritional status of patients, developing nutrition care plans and evaluate and monitoring effectiveness of nutrition interventions
  • Conduct comprehensive nutrition assessments identifying stages of change, socio-economic and cultural factors impacting health behaviours
  • Utilize appropriate counselling techniques including motivational interviewing and cognitive behavior therapy.
  • Communicate nutrition care plans and debrief with primary care team members as needed
  • Ensure ongoing nutritional care by maintaining complete and accurate client records, assessment notes and required correspondence, and by monitoring client progress on a regular basis.
  • Safeguard client records and assures confidentiality of client information.
  • Identify opportunities for quality improvement for nutrition services and interdisciplinary work.
    Work together within RD group to create resources to enhance patient access to timely, evidence-based primary care nutrition.
    Facilitate/co-facilitate groups with other staff of the FHT.

    Maintain professional competence through participation in professional development activities and maintaining license to practice.

  • 4-year Bachelor of Science or Applied Science degree, majoring in Nutritional Sciences or Human Nutrition (or equivalent); completion of Dietitians of Canada accredited Dietetic Internship program or combined Master’s and Dietetic Internship program
  • Good standing with the College of Dietitians of Ontario and possess professional liability insurance.
  • Certified Diabetes Educator (CDE) and Second language are an asset

About the Job

We are seeking a passionate Nurse Practitioner to join our dynamic team.

The ideal candidate will work closely with the family physician to provide primary health care services to patients. The main duties and responsibility will include the following:

Health maintenance: Including the care of at-risk populations; wellness exams; pap tests and breast exams; prenatal care; well-baby/child visits and immunizations. Conduct periodic examinations including use of an age/sex specific recommended preventative interventions and risk assessments; home visits for the frail elderly/geriatric patients with multiple medications and administration of immunizations.
