Building Primary Care Close to Home
In line with Ontario’s Primary Care Action Plan, at the Hamilton Family Health Team we’re collaborating with partners towards care that’s close to home and addresses the needs of the neighbours we serve.

Health Care at Eva Rothwell Centre is a small but mighty primary health care clinic located inside Eva Rothwell Centre (ERC), a vibrant community hub with wide-ranging supportive programs for children, individuals, and families in Hamilton’s North End.
As of May 2025, patients have had:

Integrating Social Medicine and Population Health
The HFHT, in partnership with Thrive Group, the Mary Heersink School of Global Health and Social Medicine at McMaster University, and the GHHN Primary Care Network, is working to develop an integrated social medicine hub and population health research lab at 304 Victoria Ave North. In collaboration with HFHT family physicians, their team members, and our on-site IHPs, the aim is to develop a unified interprofessional model of care that leverages the principles of social medicine to better understand the social determinant gaps impacting the health of the nearly 20,000 patients served at this co-located site. This work will involve embedding community connectors, academic researchers, and health professional learners into the care environment as well as developing community partnerships with non-health system stakeholders to help address population-level barriers to health.
