“Over the past year, we’ve gone through a tremendous amount of growth at the Hamilton Family Health Team, while continuing to put you, our patients, at the centre of everything we do.

As we close out one Strategic Plan and launch another, our mission has remained: to provide accessible, comprehensive and collaborative primary health care to achieve the best possible health and quality of life for our community.

As both the Chair of our Board of Directors and a family physician myself, I can confidently say it is a privilege to be able to walk alongside you on your health journey. We are also sincerely grateful to all of our Board Members, partner organisations, physician members, healthcare providers, and staff for the dedication they show up with each and every day. We are proud of our collective work this year, and excited for the future of team-based primary health care in our community.”

The City of Hamilton is situated upon the traditional territories of the Erie, Neutral, Huron-Wendat, Haudenosaunee and Mississaugas.

Today, the City of Hamilton is home to many Indigenous people from across Turtle Island (North America) and we recognize that we must do more to learn about the rich history of this land so that we can better understand our roles as residents, neighbours, partners, and caretakers.

This land is covered by the Dish With One Spoon Wampum Belt Covenant, which was an agreement between the Haudenosaunee and Anishinaabek to share and care for the resources around the Great Lakes. We further acknowledge that this land is covered by the Between the Lakes Purchase, 1792, between the Crown and the Mississaugas of the Credit First Nation.

Two ways we embody this commitment:

  1. Integrating concepts of Equity, Diversity, and Inclusion (EDI) into our
    way of being, and
  2. Being a leader in environmental stewardship in healthcare through our Green Initiative.
