Part-Time Nurse Practitioner (NP)

2-3 days or 16-24 hours weekly

Duties will include but are not limited to:

-Provide comprehensive assessments, diagnosis, and management of acute and chronic health conditions across the lifespan.
-Order and interpret diagnostic tests, prescribe medications, and develop care plans in collaboration with the patient.
-Deliver preventative care, health education, and chronic disease management.
-Collaborate closely with family physicians, dedicated nursing staff, allied health professionals, and community partners.
-Maintain accurate and timely documentation within the electronic medical record (EMR).
-Participate in quality improvement initiatives and contribute to a culture of patient safety and continuous improvement.

Qualifications:

-Nurse Practitioner: Registration in good standing with the College of Nurses of Ontario (CNO) as an NP (Primary Health Care or equivalent).
-Current certification in Basic Life Support (BLS).
-Experience in primary care, family health teams, or community health settings preferred.
-Excellent clinical judgement, communication, and interpersonal skills.
-Proficiency in using EMRs (e.g., Avaros) considered an asset.
