Our Patient Relations & Experience team will listen, respond with privacy and respect, and work with you and your care team to address your feedback.
Patient Feedback Process
Patients and their care partners are encouraged to share feedback with their care providers. This allows for direct communication and problem-solving. However, some people may feel more comfortable sharing their feedback with someone outside their care team. In such cases, the HFHT Patient Relations & Experience Office can help you.
They will work with you to address your concerns and inform you of the steps taken to resolve the issue. Here’s what happens when you share feedback with Patient Relations:
- We contact you with two business days to acknowledge receiving your feedback and gather more information.
- We seek your consent to speak with your care providers about your feedback.
- We document and share your feedback with your care providers.
- We work with your care providers to develop a plan to address your feedback, and any quality improvement needed.
- We complete any follow-up actions that were identified within 30 business days and keep you informed if it takes longer.
- We contact you to close the loop if requested.
- We share data about trends and patterns with care providers and leaders for quality improvement.
