Summary: Responsible for assessing and prioritizing patient needs within the primary care centralized hub(s). This role ensures timely access to appropriate care by evaluating symptoms and functioning, assessing social determinants of health, determining urgency, and directing patients to the most suitable healthcare services. The clinician will collaborate closely with primary care providers, interdisciplinary healthcare providers and community resources to ensure seamless patient care.
May be required to carry a small patient case load or back-up mental health coverage if capacity allows.
Schedule: Monday-Friday, 8:30 am – 4:30 pm
