Nursing Program

Who We Are 

Registered Nurses within the Hamilton Family Health Team work with our doctors, mental health counselors, dietitians and pharmacists to provide primary care that promotes health and prevents disease.

Nurses are the “gateway” to care. They use their skills to direct patients to the care that they need, from the appropriate health care provider at the right time, freeing up physician time to meet the increasing medical needs of our community.

Never before has such a large group of community family practice nurses been organized in this manner - these are exciting times marking the beginning of a new day and a new way in community nursing!

What We Do

Registered Nurses at each participating HFHT practice perform some or all of the following roles depending on the structure of their clinics:

  • Initiating the patient record, taking vital signs, and preparing patients for the physician visit.
  • Completing immunizations, allergy shots, flu shots and TB testing.
  • Ear syringing, suture removal and dressing changes.
  • Completing driver’s physicals, prenatal examinations, well woman and well child examinations.
  • Directing patients to the appropriate care provider at the right time.
  • Treating minor infections such as urinary tract infections, pink eye and yeast infections over the telephone using medical directives.
  • Responding to telephone questions regarding treatment plans and test results.
  • Monitoring patients who are on blood thinning medication.
  • Providing patient education and treatment planning for blood pressure and cholesterol management.
  • Working with other members of the health care team to develop and deliver Chronic Disease Management Programs.
