Pharmacy Program

Who We Are

The HFHT Pharmacy Program brings clinical pharmacists into the offices of participating family doctors in Hamilton, Ontario. Family doctors and pharmacists work together to ensure the most appropriate use of medications.

Program Goals

The goals of the Hamilton Family Health Team are:

to improve access to pharmacy services in primary care
to expand the range of pharmacy issues seen in primary care
to increase collaboration between family doctors and pharmacists
to increase family doctors’ knowledge and comfort managing medication issues
to strengthen links between all levels of the health care system
to support illness prevention and health promotion through education and advocacy.

How The Program Works

Clinical pharmacists with the HFHT work in the family doctors’ offices. They work directly with the family doctor and other members of the health care team.

Clinical pharmacists are able to see people with a broad range of medication issues. The first visit will review:

prescription medications
over-the-counter medications
herbal or natural remedies

The clinical pharmacist will work with you and your doctor to resolve any problems and prevent future problems. Patients see the pharmacist for one or more visits as needed, and can be re-referred at any time.
